The Comprehensive Continuity Assessment (CCA) is a complete bi-annual evaluation of your physical health, cognitive baseline, and continuity markers — delivered in a single visit to your nearest center, typically completed in under four hours.
You will arrive. You will be seen by an continuity-certified practitioner. You will leave with a complete picture of your health — and the confidence that your continuity record has been updated and secured.
Results are delivered within 24 hours to your secure Humanitas Trust health portal. Your practitioner will follow up within 48 hours to review findings and recommend next steps.
All CCA practitioners hold current continuity certification under GCI Operations Protocol 4.1. Continuity records are maintained in compliance with GCI Charter data stewardship provisions.
Every Humanitas Trust journey begins here. Baseline Health Mapping is the comprehensive onboarding assessment for new program participants — establishing the personal health baseline against which all future care will be measured, calibrated, and understood.
This extended initial visit includes everything in the Comprehensive Continuity Assessment, plus a full life-history intake, environmental factor mapping, and hereditary continuity profiling. We invest the time at the beginning so that every subsequent visit can be precise, efficient, and meaningful.
For participants who require more than an annual assessment, the Continuity Care Program provides ongoing monitoring, quarterly check-ins, and real-time continuity calibration. Your dedicated care team tracks your longitudinal record and intervenes proactively — before concerns become conditions.
Continuity Care participants receive priority scheduling at all 3,400+ global centers, access to our specialist network, and a personal continuity profile reviewed quarterly by senior continuity-certified practitioners.
The most powerful form of continuity care is shared. Family Continuity Enrollment extends all Humanitas Trust programs to every member of your household under a unified family continuity record — coordinated scheduling, shared health history where appropriate, and a single trusted relationship with your regional Continuity Center.
Family enrollment is available for households of any configuration. Children's baseline assessments are handled by our pediatric continuity specialists. Hereditary continuity mapping across generations is included at no additional cost.
